What this interaction is about
A possible influence of glucosamine on coumarin-type anticoagulants has been described. In Europe that mainly means phenprocoumon; warfarin is better known internationally. Cases have been reported in which the INR value shifted after glucosamine was added.
INR is the measure used to set anticoagulation. If it shifts, the balance between clotting and bleeding risk shifts with it. That is why this combination is named explicitly in safety guidance.

How strong the evidence is
The information rests largely on individual case reports and pharmacovigilance notifications rather than large controlled trials. That means two things: it is not automatic, and it cannot be dismissed either.
For a medicine whose dose is set individually by laboratory values, a plausible suspicion is enough to justify caution. The potential harm is significant, while doing without a supplement costs nothing.

What to do in practice
If you take an anticoagulant, do not start a glucosamine product without speaking to your prescribing practice or pharmacy first. Bring the pack or a photo of the label so the exact composition can be assessed.
Never stop a prescribed medicine on your own in order to take a supplement. If you do start after consultation, closer INR monitoring during the first weeks is the usual approach.

Supplements are often stopped as a precaution before planned surgery. Raise the appointment early and list everything you take, including herbal products.
